Nose to mouth folds and marionette lines · Hale, Cheshire

Natural cheek contour and skin at Dr Ed Robinson Clinics

Folds beside the nose and below the mouth are not simply empty lines waiting to be filled. They often become more noticeable as the surrounding face changes. We assess those changes before deciding whether a small amount of support, skin treatment or a different approach would help.

Hale clinic · Consultation first

You do not need to choose a treatment before contacting us. We start by understanding the cause, then explain whether a clinic pathway, observation or a referral is the more sensible next step.

At a glance

How we think about facial folds

Assessment focus

We examine the cheeks, chin, mouth and jawline together, including movement.

Possible pathways

Facial dermal filler, Skin resurfacing, Laxity assessment

Our approach

Understand the cause first; recommend treatment only when the likely benefit justifies the risk, recovery and cost.

Medical care first

New facial weakness, sudden asymmetry or a rapidly developing swelling requires medical assessment rather than a routine cosmetic plan.

What this concern can involve

Nasolabial folds run from the sides of the nose towards the mouth. Marionette lines extend downwards from the mouth corners. Normal anatomy and expression create these transitions even in younger faces. Ageing, changes in facial volume, dental support and skin laxity can deepen them. A surface crease over a fold is a separate skin component.

Normal anatomy

These transitions are present even in younger faces.

Facial volume

Changes in support around the cheeks and chin can deepen a fold.

Skin laxity

Looser skin can make folds more visible.

Surface crease

An etched line over a fold is a separate skin component.

How we assess it

We examine the cheeks, chin, mouth and jawline together, including movement. We ask whether the concern is the depth of the fold, a shadow, a downturned corner or the texture of the skin. Previous filler is important: a persistent line does not necessarily mean more volume is needed in that exact place.

Assessment pathway

A useful consultation for facial folds follows a clear sequence rather than jumping to a product.

1

Your concern

The depth of the fold, a shadow, a downturned corner or the texture of the skin.

2

Examination

Cheeks, chin, mouth and jawline together, including movement.

3

Previous filler

What remains, since a persistent line does not always mean more volume is needed there.

4

Plan

Support, skin treatment, a laxity assessment, a referral or leaving some folds visible.

Your consultation

You do not need to choose a treatment before contacting us. Tell us what you have noticed, when it changed, what you have already tried and what a worthwhile result would look like to you. We consider your health, medication, previous procedures, skin or tissue characteristics and practical tolerance for recovery.

A useful consultation may lead to treatment, a staged plan, a period of observation or a referral. We would rather explain that an option is unlikely to help than place you into a course simply because the treatment is available.

  • What you have noticed and when it changed
  • Which part bothers you most: depth, shadow, corners or texture
  • Previous filler in the cheeks, folds or chin
  • Medicines, health changes and recovery constraints
  • What a worthwhile result would look like to you

Options we may discuss

Folds beside the nose and mouth have more than one cause, so there is no single fix. The right option depends on what we find at consultation.

Facial dermal filler

May address a genuine support deficit or a specific local depression. Direct treatment is considered only where it has a clear purpose; filling a fold heavily can create an unbalanced, bulky lower face.

Skin resurfacing

May improve an etched skin component but does not reposition all the deeper tissues responsible for a fold. Recovery and risk depend on intensity.

Please note, Erbium laser resurfacing is only available in Hale. However, we have a guide on where to stay if you are able to travel. We promise it’s worth the visit! Planning your visit.

Laxity assessment

Can help determine whether skin tightening is worthwhile. Significant tissue descent may be more appropriately discussed with a surgeon, while some folds are best left partly visible.

Please note, Ultherapy Prime is only available in Hale. However, we have a guide on where to stay if you are able to travel. We promise it’s worth the visit! Planning your visit.

Cheek filler

Can address a local midface deficit, but cheek-only treatment may miss other changes or produce an unnatural transition. We explain why a particular area is or is not a priority.

Radiesse

Radiesse is a biostimulator that may be discussed where mild sagging or reduced skin support is part of the picture. It is not a replacement for every lost volume, and skin boosters do not lift sagging tissue. Suitability depends on your anatomy and assessment.

Morpheus8

Combines microneedling with radiofrequency energy to heat deeper layers of the skin and support collagen remodelling. It may help texture and mild laxity; recovery and suitability depend on your skin and the settings used.

What treatment may realistically achieve

A natural improvement usually preserves the normal transitions of the face. Filler does not provide the same tissue repositioning as surgery. We agree what change would be meaningful, discuss the risks and allow time for settling before judging whether another step is needed.

Comfort, safety and clinical judgement

Comfort planning depends on the procedure. It may include topical or local anaesthetic, cooling, paced treatment or a nerve block where clinically appropriate. A nerve block is not needed for every concern, and comfort measures do not remove treatment risk.

For some injectable assessments, ultrasound may help answer an anatomical question, identify existing filler or guide aspects of treatment or complication management. It is an additional clinical tool, not a guarantee against bruising, vascular occlusion or other complications. Device treatments require the same attention to diagnosis, skin type, settings, eye protection, aftercare and escalation.

Nerve blocks and comfort

Where clinically appropriate, a nerve block can form part of comfort planning for injectable pathways. It is discussed beforehand and is not needed for every nose to mouth fold concern.

Ultrasound as a clinical tool

Ultrasound can help when anatomy or residual filler is unclear. It supports clinical judgement; it does not remove the risks of treatment.

Medical thinking

Why choose Dr Ed Robinson Clinics

Our approach is medical, conservative and consultation-led. We look at how the concern fits the whole face, skin or health picture rather than treating an isolated photograph.

Cause before product

Folds reflect anatomy, support, laxity and skin. We look at the whole lower face before recommending a pathway.

What you can do at home

Sun protection and tolerated active skincare can help the skin component over time. Facial exercises, tape or creams cannot be relied on to permanently lift a structural fold. Avoid using filtered photographs as the standard for a normal face.

When to seek medical advice

New facial weakness, sudden asymmetry or a rapidly developing swelling requires medical assessment rather than a routine cosmetic plan.

Frequently asked questions

Should the line itself be filled

Sometimes a small local treatment has a role, but it is not automatic. We first assess whether surrounding support, laxity or skin texture explains the concern.

Will cheek filler remove my smile lines

It may alter some proportions, but it cannot reliably erase a structural fold. Excess cheek volume may create a new imbalance.

Are smile lines always a sign of ageing

No. They are part of normal anatomy and can be prominent in young faces. The aim is not necessarily to eliminate them.

Can laser treat marionette lines

It may help a superficial skin crease. It cannot reproduce the effect of changing deeper support or surgically repositioning descended tissue.

Why do my folds still show after filler

The remaining fold may reflect normal anatomy, laxity or skin change. More product is not necessarily useful and can increase heaviness.
