Unlike cosmetic retinol, tretinoin is already in its active form, meaning it begins working directly within the skin without requiring conversion. Because of its potency, it should only be prescribed following assessment by a qualified clinician and is generally reserved for patients who have already developed good tolerance to lower-strength retinoids.

It is typically prescribed only for patients who have successfully used lower-strength tretinoin and require a higher-strength formulation to continue progressing their results. Choosing the correct strength is about achieving the best long-term outcome while maintaining skin health, not simply using the strongest product available.

Typically, a pea-sized amount is applied to completely dry skin in the evening, avoiding the eyes, lips and corners of the nose. Even experienced users may be advised to increase usage gradually when progressing to the 0.1% strength.
Daily broad spectrum SPF is essential throughout treatment.
Higher strength doesn't necessarily mean better results. While 0.1% tretinoin is the strongest cream in the Obagi range, it also carries a greater likelihood of irritation. We only recommend progressing to this strength when your skin has already adapted to lower concentrations and your clinician feels it's appropriate. Consistency, patience and barrier support will almost always produce better long-term results than increasing strength too quickly.
